There are MANY forms to be FILLED OUT, SIGNED AND RETURNED as soon as possible. Please go to the camp website to download any additional medical forms you need if your student has special circumstances (listed in packet). If your student requires medication, please make an appoinment NOW for a doctors visit, as the form requires their signature. Any medication taken to camp MUST REMAIN IN THE ORIGINAL PACKAGING and put into a Ziploc bag along with the GREEN medical form. trueMM/DD/YYYY
